Coronary Heart Disease (CHD) remains the leading cause of global mortality, yet many patients face barriers in accessing traditional, centre-based cardiac rehabilitation (CR). Building on a previously trialled and co-developed digital health platform, ECME-CR, this solution integrates wearable technologies and validated exercise delivery protocols to support remote cardiac rehabilitation.
The ECME-CR system includes a web-based app, a data aggregation system (CABIE+/SIMS), and two connected wearable devices (Withings ScanWatch and BPM Connect). The platform’s potential lies in its ability to engage individuals less likely to attend in-person programmes.

The key objectives of Cardiac Rehab are:

1. Delivering a refined ECME-CR platform with enhanced features.
2. Providing seamless remote monitoring by integrating wearables with real-time data aggregation and feedback.
3. Enabling proven remote cardiac rehabilitation.

Expected Impact

1.Clinical: Provides an accessible, engaging platform for at-home cardiac rehabilitation—potentially improving patient adherence and outcomes.

2.Societal: Increases health equity by reaching populations unable or unlikely to attend centre-based rehab.

3.Commercial Impact: Establishes a viable digital health product in collaboration with health systems or industry.
