People living with serious illness are often required to make complex healthcare decisions without adequate preparation, leading to distress and care that may not reflect their values and preferences. Although Advance Care Planning (ACP) is designed to support these conversations, uptake remains low. Many individuals recognise its importance but do not engage because of behavioural barriers, uncertainty, and a lack of structured support. Existing digital ACP solutions focus primarily on completing forms rather than preparing people for meaningful, shared decision-making.
Co-Decide is an AI-enabled Software-as-a-Service (SaaS) platform that transforms ACP from a crisis-driven documentation exercise into a proactive, values-based, and collaborative process. Combining behavioural science, readiness assessment, guided conversational workflows, and family-inclusive pathways, the platform supports earlier and more effective engagement while improving alignment between patients, families, and clinicians. It generates structured, clinician-reviewable outputs that can be incorporated into existing care pathways, while avoiding autonomous clinical or legal decision-making. The platform is also designed to align with the emerging EU digital identity ecosystem, enabling secure, portable, and verifiable recording of care preferences across healthcare settings.
Over a 12-month programme, the project will advance from Technology Readiness Level (TRL) 2–3 to TRL 5–6 by validating the technical solution, demonstrating user and clinical value, and refining a scalable commercial model for healthcare providers and public health systems. This will establish the foundation for pilot deployments, regulatory progression, and follow-on investment.
