Multimorbidity affects more than 50 million Europeans, placing significant pressure on individuals and healthcare systems. However, existing digital health solutions remain fragmented, disease-specific and poorly suited to the complex needs of older adults and community-based care. The Pro-Advance Translational Research Project (TRP) will transform ProACT—an evidence-based digital health platform for multimorbidity management—from a validated research prototype into a fully commercial, market-ready solution.
ProACT integrates remote health monitoring, personalised feedback, engagement analytics and care-network communication within a single platform to support symptom stabilisation, coordinated triage and proactive care. Large-scale evaluations through the Horizon 2020 ProACT and SEURO projects have demonstrated high user engagement, improved symptom stability, reduced healthcare utilisation and strong health-economic value, providing a compelling foundation for commercialisation.
This TRP will advance ProACT from Technology Readiness Level (TRL) 7 to TRL 9 through coordinated commercial and technical development. Commercial activities will establish a spin-out company, secure intellectual property agreements, develop strategic technology partnerships, refine a customer-driven value proposition and build an early adopter pipeline. Technical activities will strengthen interoperability, scalability, cybersecurity and accessibility, implement priority product enhancements informed by clinical evaluations, and achieve regulatory readiness through Medical Device Regulation (MDR) and ISO-aligned development pathways.
The project will deliver the first commercially available digital health platform specifically designed to support multimorbidity management at scale, enabling more integrated, proactive and cost-effective care for ageing populations while creating a strong commercial opportunity in the rapidly growing digital health market.
